Which amount of fluid do we need to evaluate preload dependency?

چکیده
Objectives The objective of this study was to evaluate the ability of the stroke volume (SV) variation with 250 ml (Δ250SV) of volume expansion (VE) to predict further SV increase with VE. We also studied the ability of respiratory SV variation (ΔrespSV) to discriminate fluid responisvenness according to the amount of VE (250 then 500 ml). Fluid expansion consisted on infusion of 500 ml of Ringer lactate fractioned by step of 250 ml over 10 min.

Use of speckle-tracking strain in preload-dependent patients, need for cautious interpretation!
BACKGROUND In critical patients, left ventricular ejection fraction and fractional shortening are used to reflect left ventricular systolic function. An emerging technique, two-dimensional-strain echocardiography, allows assessment of the left ventricle systolic longitudinal deformation (global longitudinal strain) and the speed at which this deformation occurs (systolic strain rate). This tech...
متن کاملPassive leg raising test with minimally invasive monitoring: the way forward for guiding septic shock resuscitation?
BACKGROUND Swift and adequate fluid loading is a cornerstone of septic shock therapy. Yet, careful assessment of volume responsiveness and volume amount during the resuscitation process is a prerequisite. Both overzealous initial fluid administration and late fluid overload are harmful and may be associated with increased mortality. MAIN BODY Static (i.e., central venous or pulmonary artery o...
متن کاملArterial Pressure Variation as a Biomarker of Preload Dependency in Spontaneously Breathing Subjects – A Proof of Principle
OBJECTIVE Pulse (PPV) and systolic pressure variation (SPV) quantify variations in arterial pressure related to heart-lung interactions and have been introduced as biomarkers of preload dependency to guide fluid treatment in mechanically ventilated patients. However, respiratory intra-thoracic pressure changes during spontaneous breathing are considered too small to affect preload and stroke vo...
متن کاملPredicting the Need for Fluid Therapy—Does Fluid Responsiveness Work?
Fluid overdose can be harmful in critically ill patients. Since central venous pressure (CVP) is currently considered to be an inappropriate indicator of preload, much attention is being given to predicting fluid responsiveness, i.e., the response of stroke volume (SV) or cardiac output (CO) to fluid challenge. However, when fluid responsiveness was evaluated in critically ill patients, includi...
متن کاملRestrictive versus Liberal Fluid Resuscitation in Children with Dengue Shock Syndrome: the differences in Clinical Outcomes and Hemodynamic Parameters
Background: Fluid resuscitation is the mainstay of treatment to counteract massive plasma leakage in dengue shock syndrome. We aimed to determine the differences in clinical outcomes and hemodynamic parameters of children with dengue shock syndrome post restrictive and liberal fluid resuscitation. Materials and Methods: A retrospective observational study of pediatric patients who were betwee...
